Where am I?
CHECK LIST FOR STUDENT REGISTRATION IN ACCORDANCE WITH SCHOOL ADMISSIONS POLICY
Required:
[ ] Birth Certificate issued by Bureau of Vital Statistics [baptismal/hospital certificates are not acceptable]
[ ] Proof of parent/guardian's residency. Proof of residency may be established by providing any two of the following: unpaid utility bill, occupancy permit, rental or purchase contract, real or personal property tax bill, or IRS tax return.

[ ] Health Information Form
[ ] Residency Form
[ ] Signed statement of prior suspension/expulsion
[ ] Signed statement of prior criminal offense
[ ] Academic Records (including grades, test scores, discipline and attendance.
[ ] Special Education records (IEP, psychological/ diagnostic evaluation)
[ ] Identification of Migrant Children Form
Desired: (Not required)
[ ] Student Social Security Card
[ ] A copy of the section of the dissolution decree (not the petition) which stipulates custody in cases of parent divorce.
