How to Register a Child for School

Please call the office to make an appointment to register your child. The counselor and secretary are available to answer your questions about curriculum and the enrollment process. Registration is complete and the student may attend when all enrollment paperwork is completed and required documents have been provided.

Under Missouri school law (RSMo 167.151), a student must live within a school district in order to attend that district's public schools without charge. "The term residency shall mean that a person both physically resides within a school district and is domiciled within that district. The domicile of a minor child shall be the domicile of a parent or court-appointed legal guardian. The student and parent must provide proof of residency in the district."

These documents, provided by parent/guardian, are required for registration:

  • Birth certificate issued by Bureau of Vital Statistics (baptismal/hospital certificates are not acceptable)
  • Proof of parent/guardian residency (Real/Personal Property Tax Bill, rental/purchase contract, occupancy permit, utility bills, current voter registration card, IRS tax return
  • Immunization records (see Clinic web page for necessary immunizations)
  • Student's Social Security Card (number) is desired, not required

Paperwork provided by the office to be completed:

  • Health Information Form & Medical Consent Form
  • Registration Form
  • Residency Verification
  • Safe School Act, Forms HB1301 and 1298
  • Student Record Request Form
  • Technology Usage Form
  • Emergency Form for Child Pickup
  • Student Transportation Form

STUDENTS WITH POWER OF ATTORNEY

In addition to all the above, the following must be completed through the Office of Student Services:

  • Power of Attorney
  • Residency Verification or Waiver Form
  • Completed and signed Power of Attorney and Residency Verification or residency Waiver Form will be maintained in the Office of Student Services. Buildings will receive for their records an Official Permit.
