Immunizations
In order to attend school, students must provide proof of
immunizations, a signed Physician’s Exempt form or Religious Exempt
form upon registration to North Glendale which complies with Missouri
State Law. Please submit proof of any additional immunizations received
during the school year.
2008/2009 Immunization Requirements
Grades K-3
4+ doses of Diphtheria, Pertussis, Tetanus (last dose after 4th birthday)
3+ doses of Polio (last dose after 4th birthday)
2 doses of Measles, Mumps, Rubella
3+ doses of Hepatitis B
1 dose of Varicella (chickenpox) or proof of disease
Grades 4-5
4+ doses of Diphtheria, Pertussis, Tetanus (last dose after 4th birthday)
3+ doses of Polio (last dose after 4th birthday)
2 doses of Measles, Mumps, Rubella
3+ doses of Hepatitis B
Grades 5-8
4+ doses of Diphtheria, Pertussis, Tetanus (last dose after 4th birthday)
3+ doses of Polio (last dose after 4th birthday)
2 doses of Measles, 1 dose of Mumps, 1 dose of Rubella
