Must be turned in to your college counselor at least two weeks prior to your deadline.

The following colleges/ universities require letters of recommendation as part of my application for admission. Please electronically submit my letters, which have already been requested, to the institutions as outlined below. I understand that I am responsible for requesting these letters from my teachers and college counselor at least two weeks prior to my deadline. In addition, I have already submitted a resume/ Teacher Recommendation Request Form to my teacher and/or Counselor Recommendation Request Form to my college counselor, as needed.

Required for counselor recommendations!

Completion of this questionnaire will assist us in writing your college counselor recommendation. Your input aids us in presenting a desirable holistic view of you to the admissions office. Do your best to answer each question in a thoughtful manner. Be honest!!!!!


Provide specific information. Please do not sell yourself short- This is the time to brag about all you have accomplished! Use additional paper if needed.

  1. Describe a challenge you have faced in your life. How has this experience impacted you? (This could very well be a college essay question you will soon see!)

  1. Looking over the past several years at Kirkwood, what is your favorite subject? What classroom experiences have you valued most? Why? Be specific.

  1. How will you set yourself apart from all other applicants? What talents, characteristics and/or traits do you have that will impress a college and make you UNIQUE?



  1. Is there anything unique about your family that you would like to share?



  1. What are the first words that come to mind to describe you? Or if this is too tough, how would your friends and family describe you?



  1. What three things would you like the College Admissions Committee to know about you?



  1. Why are you applying to these particular colleges? (Be specific if something stands out that influenced your decision to apply.)
