MEDICATIONS
Written authorization is required for any medication to be given. Medication must be in their original container. For prescription medications, it must be labeled with student’s name and pharmacy label. For over-the-counter medication, it must be in original container.
All medications need to be delivered to clinic by parent/guardian.
We do not have a stock supply of over-the-counter medications to give to your child. If your child has medication here in the clinic, and you give him/her a dose of that medication in the morning before school, please let RN know. If we are not informed, there is potential for accidental overdose. We will let you know by phone call or note that your child was given a dose of over-the-counter medication and the time it was given.
We have limited space to hold medication. As much as I like to be prepared, there may be times where I cannot keep all OTC meds here in clinic. I will let you know if I send them home with your child.
Again, written authorization is needed for any and all medication that is given to your child. You may download a copy of the form here.
