I've attached some forms to be downloaded and completed if needed.
If you are going to send medication to school with your child. Please send medication in original container. Check the expiration date on medication.
Download medication form, complete form. I prefer that the parent/guardian drop off the medication to the clinic. If that is not an option that morning, please call to let me know that your child has a medication to bring to clinic.
DO NOT SEND ANY CONTROLLED SUBSTANCE TO SCHOOL WITH YOUR CHILD.
That medication HAS to be brought in by a parent/guardian.
The other forms that I've put on this page are Physical Forms, Asthma questionairre and a diabetic history form. This page will be updated as more
information becomes available.
