Immunizations Required for Children in Missouri Child Care & Preschools
Students must have received these vaccines by the first day of school unless the doctor has signed a form stating the immunizations are in progress. Documentation of these immunizations or the form must be on file by the first day of school. Forms may be obtained in the nurse's office.
Exceptions:
1. If a child has had chickenpox, a written, signed statement by the doctor giving the month and year of the illness must be on file and no immunization will be required.
2. A medical exemption in writing may be given by your child's doctor. Forms are available in the nurse's office or the doctor may write the exemption on his own stationery.
3. A religious exemption must have a "religious exemption" form on file with the school. You may get this form from the school nurse.
Please note: Your child may not attend school if there is an outbreak of the disease for which your child is exempt.
